(f) Posttraumatic stress disorder.
(g) Amyotrophic lateral sclerosis.
(h) Crohn’s disease.
(i) Parkinson’s disease.
(j) Multiple sclerosis.
(k) Medical conditions of the same kind or class as or comparable to those enumerated in paragraphs (a)-(j).
(l) A terminal condition diagnosed by a physician other than the qualified physician issuing the physician certification
(m) Chronic nonmalignant pain.

By law, you must see a physician every 7 months for a face-to-face evaluation. And you must renew your card with the state each year. The costs may vary by physician, but at FL Medical Cannabis we save you the hassle and come to your home for the evaluation. If you have never had a Florida medical marijuana card the cost is $219. If you are switching from a different physician, or for your first 7 month renewal the cost is $169. The cost of the second and all other renewals is $109. In addition, you will need to pay the state of Florida $75 for your initial card and $75 to renew it each year.
